ACCELERATED DEATH BENEFIT
This valuable feature allows you to collect half of your benefit amount up to $250,000 at a time when you’ll need it most. If you’re diagnosed with a terminal illness and given a life expectancy of 12 months or less, you can access your cash benefits to use any way you see fit.
This added cash can be used to help pay medical bills … or to cover household expenses so a family member can stop working to care for you. The remainder of your benefit will then be paid to your beneficiary.
Please note that the receipt of Accelerated Death Benefits may affect your eligibility for public assistance programs and may be taxable. Prior to applying to receive such benefits, you should consult with the appropriate social services agency and seek the advice of a qualified tax advisor.

WHY GROUP 10-YEAR LEVEL TERM LIFE?
Many families are looking for long-term financial security they can depend on. This plan allows members and their spouses to lock in member exclusive rates and benefit amounts for 10 years. This makes it easy to fit within your busy lifestyles, as well as budget from year to year with the guarantee of no rate increases.
WHO CAN HAVE THE 10-YEAR LEVEL TERM LIFE COVERAGE?
CSEA members who are under age 65, U.S residents, actively employed working at least 20 hours per week (excluding Retired Members) can apply for coverage. Evidence of Insurability is required.
Retired CSEA members are still eligible for coverage provided they are under the age of 65, and remain a Retired CSEA member. Evidence of Insurability is required.
You must not be on active duty with the military.
HOW MUCH COVERAGE AM I ELIGIBLE FOR?
Members are eligible for a minimum coverage amount of $100,000 and can increase in increments of $10,000 up to a maximum of the lesser of $1,000,000 or 10 times your yearly salary.
The aggregate amount of coverage for all combined CSEA plans is 1 million for the member and policy regulated amounts for their spouse.
CAN I ALSO REQUEST COVERAGE FOR MY FAMILY?
Yes. You can apply to have your spouse covered for the same coverage, but it cannot exceed you, the member’s, coverage. They too can start at $100,000 and add on increments of $10,000 up to a maximum amount of the lesser of $1,000,000.
CONVENIENT PAYROLL DEDUCTION:
WHEN DOES MY COVERAGE BECOME EFFECTIVE?
After your application is approved, coverage will start the first day of the month following the first payment, if you are performing the normal activities on that date. If not, coverage starts the day you return to work.
Example: If your application is approved on May 5, and your first premium is deducted from your June 1 pay warrant, then coverage will start on June 1 if you are at work on that date.
WHAT HAPPENS TO MY COVERAGE WHEN I TRULY RETIRE?
When you truly retire, you can continue your life insurance if; you receive monthly California Public Employees Retirement System (CalPERS) benefits from which premiums may be deducted; you continue your CSEA membership; and you send in an application requesting benefits to be continued into retirement before your retirement date and continue premium payments thereafter.
CAN I REQUEST OTHER CSEA SPONSORED LIFE INSURANCE PROGRAMS?
Yes. You may be eligible to apply for up to a million dollars of life insurance coverage. CSEA offers additional Group TermPLUS Life and Group Whole Life insurance plans. Please note that the combined amounts of all Group TermPLUS Life coverage, all Group Whole Life Coverage, and all 10-Year Level Term Life coverage in effect for the member cannot exceed $1,000,000. Terms
Effective Date of Coverage
Your CSEA Group 10 Year Level Life benefits can begin following approval from New York Life. Your coverage will take effect on the first day of the month after your application has been approved and first payment has been received, via payroll deduction or from CalPERS if you are retired. Once approved you will receive a Certificate of Insurance describing the terms and conditions of your coverage. If you are not working full-time on the date your insurance is scheduled to become effective, your insurance will not become effective until you return to full-time work. However, if you are not actively working due to a non-working day, you will still become insured on the scheduled effective date provided you would have been able to do your regular job on that non-working day.
When Coverage Ends
Providing your premium is paid, you remain a CSEA member and the Group Policy remains in effect your guaranteed premium term will be for a term of 10 Years.
After your guaranteed term ends you can continue your coverage to age 90 using the non-guaranteed rates. There are other options available. Your administrator will instruct you prior to your term ending regarding your choices.
At the end of your 10 year guaranteed term, if you are a CSEA member and under age 65 you have the option of:
- Applying for another 10 Year Level Term at your then current age. Full medical underwriting is required.
- Using the CSEA Group TermPlus Life’s Exchange Provision which will allow you to exchange a maximum of 30% (up to a maximum of $300,000) of your Level Coverage for the Group TermPlus coverage (G30380-0) without medical evidence.
DEFINITIONS:
Beneficiary Designation
You can name any beneficiary you wish for member coverage. You may change this beneficiary at any time, by written request. Your beneficiary is the person(s) last designated by you in writing, and recorded by or on behalf of New York Life Insurance Company. You are the automatic beneficiary for dependent insurance, as described in the Certificate of Insurance. If you wish to name another beneficiary for dependent spouse or child insurance, contact the administrator at 1-800-296-2732 for the applicable form.
Life Coverage Exclusions
In the case of death due to suicide, an attempt at suicide or intentionally injuring oneself within 12 months of coverage, the benefit will be limited to the return of premiums paid.

New York Life Insurance Company reserves the right to request medical information to determine applicant’s medical eligibility for coverage. Based on the age of the person proposed for insurance and the amount of coverage requested, a physical examination, EKG, blood test or other information may be required. Not all applicants will have to supply additional information. However, if it is required, New York Life will arrange for an independent professional paramedic to contact you to perform these simple tests at your convenience. The exam and blood test are free-of-charge.
If we can provide the coverage you requested, we will inform you as to when such coverage will be effective. Under no circumstances will coverage be effective prior to this date. Payment of a premium contribution with your application does not mean that there is any insurance in force before the effective date as determined by New York Life.
IMPORTANT NOTICE
How New York Life Obtains Information and Underwrites Your Request for Group 10-YR Level Term Life Insurance
In this notice, references to “you” and “your” include any person proposed for insurance. Information regarding insurability will be treated as confidential. In considering whether the person(s) in your request for insurance qualify for insurance, we will rely on the medical information you provide, and on the information you AUTHORIZE us to obtain from your physician, other medical practitioners and facilities, other insurance companies to which you have applied for insurance and MIB, Inc. (“MIB”). MIB is a not-for-profit organization of insurance companies, which operates an information exchange on behalf of its members. If you apply for life or health insurance coverage or a claim for benefits is submitted to an MIB member company, medical or non-medical information may be given to MIB, and such information may then be furnished by MIB, upon request, to a member company.
Your AUTHORIZATION may be used for a period of 24 months from the date you signed the application for insurance, unless sooner revoked. The AUTHORIZATION may be revoked at any time by notifying New York Life in writing at the address provided. Your revocation will not be effective to the extent New York Life or any other person already has disclosed or collected information or taken other action in reliance on it, or to the extent that New York Life has a legal right to contest a claim under an insurance certificate or the certificate itself. The information New York Life obtains through your AUTHORIZATION may become subject to further disclosure. For example, New York Life may be required to provide it to insurance, regulatory or other government agencies. In this case, the information may no longer be protected by the rules governing your AUTHORIZATION.
MIB and other insurance companies may also furnish New York Life, its subsidiaries or the Plan Administrator with non-medical information (such as driving records, past convictions, hazardous sport or aviation activity, use of alcohol or drugs, and other applications for insurance). The information provided may include information that may predate the time frame stated on the medical questions section, if any, on this application. This information may be used during the underwriting and claims processes, where permitted by law.
New York Life may release this information to the Plan Administrator, other insurance companies to which you may apply for life and health insurance, or to which a claim for benefits may be submitted and to others whom you authorize in writing, however, this will not be done in connection with test results concerning Acquired Immune Deficiency Syndrome (AIDS) or Human Immunodeficiency Virus (HIV). We may also make a brief report of your protected health information to MIB, but we will not disclose our underwriting decision
New York Life will not disclose such information to anyone except those you authorize or where required or permitted by law. Information in our files may be seen by New York Life and Plan Administrator employees, but only on a “need to know” basis in considering your request. Upon receipt of all requested information, we will make a determination as to whether your request for insurance can be approved.
If we cannot provide the coverage you requested, we will tell you why. If you feel our information is inaccurate, you will be given a chance to correct or complete the information in our files. Upon written request to New York Life or MIB, you will be provided with non-medical information. Generally, medical information will be given either directly to the proposed insured or to a medical professional designated by the proposed insured. Your request is handled in accordance with the Federal Fair Credit Reporting Act procedures. If you question the accuracy of the information provided by MIB, you may contact MIB and seek a correction.
